Functional Yoga Medicine Certification Program is a 12-month intensive training program designed to equip you with an understanding and application of a multi-disciplinary approach to holistic healing, integrating Yoga with applied anatomy and physiology, biomechanics, nutrition, functional medicine, transformational coaching, energy work, breathwork, and chakra balancing.
Description of services provided includes the following:
A) Delivery of education and information related to Functional Yoga Medicine Certification training:
- Module 1: Foundation – Laying the Groundwork for Holistic Healing
- Module 2: Core – The Essence of Physical, Mental, Emotional Stability
- Module 3: Applied Anatomy and Physiology in Yoga
- Module 4: Polarity – Tapping Into Personal Power
- Module 5: Breath – Bridging Breathwork and Self Love
- Module 6: Alignment – Reconnect With Your Truth and Deepen Mind-Body Connection
- Module 7: Balance – Physical and Mental Harmony for Inner Guidance
- Module 8: Transformational Coaching for Effective Behavior Change
- Module 9: Embodiment – Tapping Into Your Higher Power
- Module 10: Practical Application and Holistic Healing Strategies
- Module 11: Advanced Topics In Holistic Integration
- Module 12: Intuitive Eating and Real Life Integration
B) Access to the Functional Yoga Medicine Certification Program student portal, which houses all program content.
C) Guidance utilizing the Functional Yoga Medicine Certification Program implementation tools, which include Yoga instruction using the Functional Yoga Medicine pillars, intuitive eating guidelines, breathwork, meditations, visualization exercises, journal prompts, and coaching techniques.
D) Access to the Functional Yoga Medicine Certification Program private Facebook group for support and accountability.
E) 12 two-hour group Q&A virtual calls to help students understand the program content by allowing them to ask questions and get coaching from Dr. Connie.
F) 300-Hr Registered Yoga Teacher certificate, upon successful completion of the program.
G) Functional Yoga Medicine Certified Practitioner certificate, upon successful completion of the program.
H) Email support for technical questions Monday-Friday, 9 AM-5 PM ET.
You agree to indemnify and hold [Business Name], its affiliates, partners, and employees harmless from any claims, damages, liabilities, and expenses (including attorney’s fees) arising from your use of the website, products, or services or your violation of these Terms.
The client understands and agrees that using all program components, including the student portal, is conditioned upon the Client’s agreement to the Terms and Conditions for use provided by the Saerom Physical Therapy DBA Functional Yoga Medicine ™.
The client also understands and agrees that the schedule for Q&A calls with other students of the Functional Yoga Medicine™ Certification Program is subject to change with or without notice to the Client and shall be set forth by the Company. The coaches assigned to the calls shall vary based on the company’s determination and discretion.
This agreement is valid as of the date the Client enrolls and will be in effect for 12 months once the program has started.
Payment must either be made in full at the beginning of the term, which is $12,500, or, upon approval and agreement by the Company, on a monthly basis. The applicable fee for the 12-month Term for this Agreement is six (6) equal payments of $2187 or twelve (12) equal payments of $1197.
If the Client elects to finance the 12-month fee and make monthly payments, the Client authorizes the Company to charge the credit card provided and on file for the Client each month. The client also agrees to give a back-up credit card payment if the payment still needs to be processed or declined. Client acknowledges, understands, and agrees that failure to authorize the automatic payments and if Client disputes a credit card payment processing as outlined in this provision, such failure to approve or dispute shall be considered a breach of the Agreement by Client.
While the Saerom Physical Therapy DBA Functional Yoga Medicine ™ will strive to assist the Client with the highest and best level of services provided in the scope of the Agreement, the Client understands that with any program, the Saerom Physical Therapy DBA Functional Yoga Medicine ™ is unable to make promises or guarantees concerning any outcome from participation in the program. Therefore, Saerom Physical Therapy DBA Functional Yoga Medicine ™ does not guarantee success or any specific results associated with the services. Client further understands that ultimately, Client is responsible for his or her career success and that this program is designed to enhance, supplement, and support Client in his or her efforts to guide others on a holistic healing journey utilizing yoga.
The client understands and agrees that the Saerom Physical Therapy DBA Functional Yoga Medicine ™’s services are not counseling services and are not a substitute for professional counseling by a licensed psychotherapist or other licensed professional. The client understands that the Company does not consist of any licensed psychotherapists or counselors.
The client understands that the scope of services provided by Saerom Physical Therapy DBA Functional Yoga Medicine ™ only includes those services particularly described in this agreement. The limitations listed in this section are not meant to be a comprehensive list of all limitations. In addition to the restrictions above, the only services to be provided are those outlined in this agreement.
The Functional Yoga Medicine™ Certification Program contains general information about medical conditions and treatments. It is not advice and should not be treated as such.
The medical information in the Functional Yoga Medicine™ Certification Program is provided without any representations or warranties, express or implied. Functional Yoga Medicine Certification Program makes no representations or warranties about the medical information on the Functional Yoga Medicine™ Certification Program.
You must not rely solely on the Functional Yoga Medicine™ Certification Program as an alternative to medical advice from doctors or other professional healthcare providers in working with individuals.
If you have any specific questions about any medical matter, the individual should consult his or her doctor or other professional healthcare provider. If you think the individual may be suffering from any medical condition, you should encourage them to seek immediate medical attention.
You should never delay seeking medical advice, disregard medical advice, or discontinue medical treatment because of the Functional Yoga Medicine™ Certification Program.
Functional Yoga Medicine™ Certification Program Refund Policy
A refund may be provided if the Client completes all 12 months of the Functional Yoga Medicine™ Certification Program yet fails to see any results.
Successful completion of the Functional Yoga Medicine™ Certification Program includes:
- Attendance of all 12 group Q&A sessions, whether live or replay
- Proof of having watched Q&A replay calls if calls cannot be attended live (taking notes regarding the discussion in the calls, for example)
- ALL written homework assignments completed and turned in
- ALL module lessons in the student portal watched and completed
- ALL benchmark exams or assessments completed with a passing grade
- Results achieved during the Functional Yoga Medicine™ Certification Program may vary, including:
- Reduced or eliminated physical symptoms (including bloating, gas, weight gain, fatigue, constipation, diarrhea, etc.) for the Client
- Increased positive state of being (such as feelings of peace, joy, contentment, confidence, happiness, etc.) for the Client
- Decreased negative state of being (such as feelings of stress, fear, guilt, shame, anger, etc.) for the Client
- Attainment of any information that can be applied to plans of treatment when working with individuals
Refunds shall only be granted if the Client completes the Functional Yoga Medicine™ Certification Program, as delineated herein. If the Client, for any reason, finds it necessary to terminate their participation in the Functional Yoga Medicine™ Certification Program during the 12-month Term and is unable to fulfill the conditions above for successful completion, a refund shall be precluded. Should the Client opt to finance the 12-month fee for the Functional Yoga Medicine™ Certification Program, they shall remain fully liable for the total fee amount, notwithstanding their inability to partake in the program. If the Client, for any reason, elects to resign from the Functional Yoga Medicine Certification Program during the 12-month Term, they shall have the opportunity to reenroll in the subsequent iteration of the Functional Yoga Medicine™ Certification Program and complete their certification, provided that their 12-month fee has been paid in full.
To initiate a refund claim, the Client is required to dispatch an email to hello@drconniecheung within seven (7) days of the program’s termination, accompanied by substantiating documentation as delineated herein demonstrating the successful completion of the program yet an absence of favorable outcomes.
